Job summary

Hearthside Nursing Home in State College, PA is seeking a compassionate Physical Therapist (PT) to join our in-house rehabilitation team. This full-time role focuses on evaluating residents' mobility and developing individualized treatment plans to improve function and quality of life.

New graduates are welcome to apply. You will collaborate with an interdisciplinary care team, document progress, and help residents regain independence through skilled, resident-centered therapy.

Qualifications

• Active Physical Therapy license in PA, in good standing.

• Skilled nursing or long-term care experience preferred.
• Strong clinical, communication, and organizational skills.
• Knowledge of the Net Health Electronic Documentation System preferred.

Responsibilities

• Evaluate residents’ mobility, strength, balance, and functional needs
• Develop and implement individualized physical therapy treatment plans
• Provide skilled therapy to reduce pain, restore function, and promote healing
• Monitor resident progress and adjust treatment plans as needed
• Educate residents, families, and caregivers
• Collaborate with the interdisciplinary care team
• Maintain accurate, timely, and compliant documentation
• Ensure services meet all professional, legal, and regulatory standards
