Job summary

Embassy of Heartside in State College, PA, is seeking a compassionate full-time Physical Therapist Assistant (PTA) to join our in-house rehabilitation team. New graduates are welcome to apply and you will work under the supervision of a Physical Therapist to help residents improve mobility, strength, function, and independence.

Qualifications

  • Active Physical Therapist Assistant license in PA, in good standing
  • New graduates welcome to apply
  • Skilled nursing or long-term care experience preferred
  • Strong clinical, communication, and organizational skills
  • Knowledge of Net Health Electronic Documentation System preferred

Responsibilities

  • Provide skilled physical therapy treatments under the supervision of a Physical Therapist
  • Implement individualized plans of care to reduce pain and improve function
  • Monitor and document resident progress, including strength, mobility, and motor performance
  • Communicate changes in resident condition to the Physical Therapist and care team
  • Educate residents, families, and caregivers
  • Collaborate with the interdisciplinary care team
  • Maintain accurate, timely, and compliant documentation
  • Follow all professional, legal, and regulatory standards
