Job summary

Stroudsburg Post Acute Nursing and Rehabilitation in Stroudsburg, PA is seeking a PRN Physical Therapist to support residents in a subacute and long-term care setting, helping improve mobility and independence.

You will collaborate closely with physicians, nurses, occupational and speech therapists, document progress in the EMR, and participate in care plan meetings. New grads are welcome and mentorship is provided; supervision of PTAs may be required, reporting to the Director of Rehabilitation.

Qualifications

• Active Physical Therapist license in Pennsylvania (or eligible)
• New grads welcome with mentorship and training
• Experience in SNF/geriatrics is a plus
• Strong communication and interpersonal skills
• Ability to collaborate in an interdisciplinary team

Responsibilities

• Evaluate and treat residents in subacute and long-term care settings
• Develop personalized treatment plans to improve strength, balance, gait, and mobility
• Collaborate with physicians, nurses, OT, and SLP
• Document progress using electronic medical records
• Participate in care plan meetings and family conferences
• Provide compassionate, patient-centered care in a team environment
• Supervise PTAs as needed
• Report to the Director of Rehabilitation

What You’ll Do

• Evaluate and treat residents in subacute and long term care setting with a variety of orthopedic, neurological, and post-surgical conditions
• Develop personalized treatment plans to improve strength, balance, gait, and functional mobility
• Collaborate closely with physicians, nurses, occupational therapists, and speech therapists
• Document patient progress accurately and in a timely manner using our electronic medical records system
• Participate in care plan meetings and family conferences
• Provide compassionate, patient-centered care in a supportive team environment
• Supervise Physical Therapist Assistants (PTAs) as needed
• Report directly to the Director of Rehabilitation

What We’re Looking For
