Job summary

Embassy of Huntingdon Park is seeking a compassionate Physical Therapist (PT) for part-time and PRN shifts in Huntingdon, PA. You will evaluate residents, develop individualized therapy plans, and work with an interdisciplinary team to improve mobility and independence.

Qualifications

• Active Physical Therapy license in PA, in good standing.
• Skilled nursing or long-term care experience preferred.

• Knowledge of the Net Health Electronic Documentation System preferred.

Responsibilities

• Evaluate residents’ mobility, strength, balance, and functional needs.
• Develop and implement individualized physical therapy treatment plans.
• Provide skilled therapy to reduce pain, restore function, and promote healing.
• Monitor resident progress and adjust treatment plans as needed.
• Educate residents, families, and caregivers.
• Collaborate with the interdisciplinary care team.
• Maintain accurate, timely, and compliant documentation.
• Ensure services meet all professional, legal, and regulatory standards.
