Job summary

Premier Washington Rehabilitation & Nursing Center in Washington, PA, seeks a compassionate, motivated, and dedicated Full-Time Physical Therapist (PT) to join its in-house rehabilitation department. You will evaluate residents, develop individualized plans, and work with an interdisciplinary team to improve patient outcomes.

Responsibilities include documenting with Net Health EMR, collaborating with physicians and staff, and ensuring regulatory compliance.

Qualifications

  • Graduate of an accredited Physical Therapy program.
  • Current Physical Therapist (PT) license in Pennsylvania.
  • Experience in SNF or long-term care preferred, new grads welcome.

  • Evaluate residents and develop individualized treatment plans.
  • Provide evidence-based PT services to skilled nursing and long-term care residents.
  • Collaborate with physicians, nursing staff, rehab professionals and team members to optimize outcomes.
  • Document evaluations, treatments, progress notes and discharge summaries in Net Health EMR.
  • Participate in care conferences and communicate progress with families and team members.
  • Ensure compliance with state, federal, and company regulations.

Premier Washington Rehabilitation & Nursing Center is seeking a compassionate, motivated, and dedicated Full-Time Physical Therapist (PT) to join our growing in-house rehabilitation department. If you're passionate about helping patients regain independence and improve their quality of life while working alongside a collaborative interdisciplinary team, we'd love to hear from you!

What You'll Do
  • Evaluate residents and develop individualized treatment plans.

  • Provide evidence-based physical therapy services to skilled nursing and long-term care residents.

  • Collaborate with physicians, nursing staff, rehabilitation professionals, and interdisciplinary team members to optimize patient outcomes.

  • Accurately document evaluations, treatment sessions, progress notes, and discharge summaries using Net Health electronic medical records.

  • Participate in care conferences and communicate patient progress with families and team members.

  • Ensure compliance with all state, federal, and company regulations.

Qualifications
  • Current Physical Therapist (PT) license in the Commonwealth of Pennsylvania.

  • Graduate of an accredited Physical Therapy program.

  • Skilled nursing facility (SNF) or long-term care experience preferred, but new graduates are encouraged to apply.

  • Experience with Net Health EMR is a plus.

  • Strong communication, organization, and time management skills.

  • Passion for providing exceptional patient-centered care.
