Job summary

Huntingdon Park Nursing Home in Huntingdon Park, PA, is seeking a compassionate Part-time/PRN Physical Therapist (PT) to join our in-house rehabilitation team. New graduates are welcome to apply and will work to improve residents’ mobility, balance, and independence.

You will evaluate residents’ needs, develop individualized treatment plans, and provide skilled therapy to reduce pain and restore function, while collaborating with the care team and maintaining compliant documentation.

Qualifications

• Active Physical Therapy license in PA, in good standing.
• Skilled nursing or long-term care experience preferred.
• Strong clinical, communication, and organizational skills.
• Knowledge of Net Health Electronic Documentation System preferred.

Responsibilities

• Evaluate residents’ mobility, strength, balance, and functional needs.
• Develop and implement individualized physical therapy treatment plans.
• Provide skilled therapy to reduce pain, restore function, and promote healing.
• Monitor resident progress and adjust treatment plans as needed.
• Educate residents, families, and caregivers.
• Collaborate with the interdisciplinary care team.
• Maintain accurate, timely, and compliant documentation.
