Job summary

Mount Carmel Senior Living is seeking a Part-Time PRN Physical Therapist to join our in-house rehabilitation team in a SNF/LTC setting. The role focuses on evaluating residents, developing individualized plans, and delivering skilled PT services to help residents regain mobility and independence.

We value collaboration with nursing and interdisciplinary teams, with emphasis on accurate Net Health and PCC documentation and resident-centered care.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Pennsylvania Physical Therapist license.
• Previous clinical experience as a Physical Therapist.
• SNF/LTC experience preferred.
• Strong communication and teamwork skills.
• A passion for providing quality care to seniors.
• Familiarity with Net Health and PCC preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy services in a SNF/LTC setting.
• Help residents improve mobility, strength, balance, and functional independence.
• Collaborate with nursing, therapy, and interdisciplinary team members.
• Maintain accurate and timely documentation using Net Health and PCC.
• Deliver compassionate, resident-centered care.
