Job summary

Mount Carmel Senior Living Community is seeking a compassionate PRN Occupational Therapist to join our in-house rehabilitation team in Mount Carmel, PA. This SNF/LTC setting focuses on helping residents regain independence and improve daily living skills.

The role requires an OT with PA license, hospital or SNF/LTC experience, and the ability to work independently yet collaboratively within a supportive team. Net Health and PCC are used for documentation.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Pennsylvania Occupational Therapist license.
• Previous experience as a licensed Occupational Therapist required.
• SNF/LTC experience strongly preferred.
• Familiarity with Net Health and PCC preferred.
• Strong clinical judgment, communication, and organizational skills.
• Ability to work independently and collaboratively.

Responsibilities

• Perform comprehensive occupational therapy evaluations and assessments.
• Develop and implement individualized treatment plans.
• Provide skilled occupational therapy services to improve residents' independence with ADLs.
• Address functional mobility, upper extremity strength, coordination, and cognitive skills.
• Recommend adaptive equipment and compensatory strategies as appropriate.
• Collaborate with nursing, therapy staff, and interdisciplinary team members.
• Maintain accurate and timely documentation using Net Health and PCC.
• Promote resident safety, independence, and quality of life.

What We’re Looking For
