Job summary

Mount Carmel Senior Living is seeking a PRN Occupational Therapist (OT) to join our in-house rehabilitation team serving residents in a Skilled Nursing Facility / Long-Term Care setting in Mount Carmel, PA. The role emphasizes flexible scheduling and meaningful, resident-centered care.

You will perform OT evaluations, develop individualized treatment plans, and help residents regain independence in daily living activities while using Net Health and PCC for documentation.

Qualifications

• Graduate of an OT program and licensed in PA.
• Current Pennsylvania Occupational Therapist license.
• Previous OT experience required.

Responsibilities

• Perform comprehensive occupational therapy evaluations and assessments.
• Develop and implement individualized treatment plans.
• Provide skilled OT services to improve residents' ADLs.
• Address functional mobility, upper extremity strength, coordination, and cognitive skills.
• Recommend adaptive equipment and compensatory strategies as appropriate.
• Collaborate with nursing, therapy staff, and interdisciplinary team members.
• Maintain accurate and timely documentation using Net Health and PCC.
• Promote resident safety, independence and quality of life.

What You'll Do

What We’re Looking For

• Graduate of an accredited Occupational Therapy program




• Current Pennsylvania Occupational Therapist license




• Previous experience as a licensed Occupational Therapist required




• SNF/LTC experience strongly preferred




• Familiarity with Net Health and PCC preferred




• Strong clinical judgment, communication, and organizational skills




• Ability to work independently and collaboratively




• A passion for providing compassionate, resident-centered care
