Job summary

Family Home Health Network is seeking a Physical Therapist to deliver home-based PT services in the Chicagoland area. The role includes assessment, plan-of-care development, and patient/caregiver instruction to improve mobility and function.

The ideal candidate will hold an Illinois Physical Therapist license, CPR certification, and a valid IL driver’s license with auto insurance. Home Care Home Base experience is preferred, and flexibility with scheduling is highlighted in the posting.

Qualifications

• Baccalaureate or master's program in physical therapy approved by an accredited organization
• Current Illinois license as a Physical Therapist
• Current CPR card
• Valid IL driver's license and auto insurance policy
• Home Care Home Base experience preferred

Responsibilities

• Provide physical therapy services to patients, including evaluating therapeutic/rehabilitative status and contributing to the plan of care.
• Assess muscle strength, mobility, and gait to guide therapy.
• Instruct patients, families and caregivers in use and care of therapeutic appliances.
• Determine priority needs for physical therapy and coordinate with the care team.

The Physical Therapist is responsible for providing physical therapy services to patients including but will not be limited to assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care; assessing for muscle strength, mobility, gait. The Physical Therapist will also instruct patients, families and caregivers in the use and care of therapeutic appliances and determine priority needs for physical therapy.

Qualifications:
