Job summary

Deer Meadows Rehabilitation Center in Philadelphia, PA is seeking a Full-Time Physical Therapist (PT) to join our in-house rehabilitation team. You will evaluate residents, develop individualized physical therapy treatment plans, and provide skilled services to short-term and long-term care residents.

We emphasize mobility, independence, and collaboration with physicians, nursing staff, and the interdisciplinary team to ensure optimal patient outcomes.

Qualifications

• Current Physical Therapist license in Pennsylvania.
• Graduate of an accredited Physical Therapy program.
• SNF or Long-Term Care experience preferred; new grads welcome.
• Experience with Net Health and PCC is a plus.
• Strong communication, clinical reasoning, and organizational skills.
• Ability to collaborate in a fast-paced rehabilitation environment.

Responsibilities

• Establish functional goals that promote mobility, strength, balance, endurance, and independence.
• Collaborate with physicians, nursing staff, and the interdisciplinary team to ensure optimal patient outcomes.
• Educate residents and families regarding treatment plans, mobility techniques, and discharge recommendations.
• Complete accurate documentation using Net Health and PCC.
• Maintain compliance with all federal, state, and company regulations.

Deer Meadows Rehabilitation Center is seeking a compassionate and motivated Full-Time Physical Therapist (PT) to join our in-house rehabilitation team in Philadelphia, Pennsylvania. If you're passionate about helping patients regain mobility, maximize independence, and improve their quality of life, we invite you to become part of our collaborative therapy team.

Our therapists work closely with nursing, physicians, and interdisciplinary staff to provide outstanding, patient-centered care in a supportive Skilled Nursing and Rehabilitation environment.

Position Responsibilities

• Skilled Nursing Facility (SNF) or Long-Term Care experience preferred, but new graduates are encouraged to apply.

• Experience with Net Health and PointClickCare (PCC) is a plus.

• Strong communication, clinical reasoning, and organizational skills.

• Ability to work collaboratively in a fast-paced rehabilitation environment.
