Job summary

Premier Washington Rehabilitation & Nursing Center in Washington, PA is seeking a dedicated Full-Time Physical Therapist to join our in-house rehabilitation department. You will assess residents, design individualized treatment plans, and deliver evidence-based therapy to support mobility and independence.

Work with a collaborative interdisciplinary team to optimize patient outcomes, document care in Net Health EMR, and participate in care conferences.

Qualifications

• Current Physical Therapist (PT) license in Pennsylvania.
• Graduate of an accredited Physical Therapy program.
• SNF or long-term care experience preferred, but new graduates are encouraged to apply.
• Experience with Net Health EMR is a plus.
• Excellent communication, organization and time management skills.
• Commitment to patient-centered care.

Responsibilities

Premier Washington Rehabilitation & Nursing Center is seeking a compassionate, motivated, and dedicated Full-Time Physical Therapist (PT) to join our growing in-house rehabilitation department. If you're passionate about helping patients regain independence and improve their quality of life while working alongside a collaborative interdisciplinary team, we'd love to hear from you!

What You'll Do

• Evaluate residents and develop individualized treatment plans.
• Provide evidence-based physical therapy services to skilled nursing and long-term care residents.
• Collaborate with physicians, nursing staff, rehabilitation professionals, and interdisciplinary team members to optimize patient outcomes.
• Accurately document evaluations, treatment sessions, progress notes, and discharge summaries using Net Health electronic medical records.
• Participate in care conferences and communicate patient progress with families and team members.
• Ensure compliance with all state, federal, and company regulations.

Qualifications

• Current Physical Therapist (PT) license in the Commonwealth of Pennsylvania.
• Graduate of an accredited Physical Therapy program.
• Skilled nursing facility (SNF) or long-term care experience preferred, but new graduates are encouraged to apply.
• Experience with Net Health EMR is a plus.
