Deer Meadows Rehabilitation Center in Philadelphia, PA is seeking a Full-Time Physical Therapist (PT) to join our in-house rehabilitation team. The role focuses on evaluating residents, developing individualized therapy plans, and enabling mobility and independence in a SNF/LTC setting.

Deer Meadows Rehabilitation Center is seeking a compassionate and motivated Full-Time Physical Therapist (PT) to join our in-house rehabilitation team in Philadelphia, Pennsylvania. If you're passionate about helping patients regain mobility, maximize independence, and improve their quality of life, we invite you to become part of our collaborative therapy team.
Our therapists work closely with nursing, physicians, and interdisciplinary staff to provide outstanding, patient-centered care in a supportive Skilled Nursing and Rehabilitation environment.
Evaluate residents and develop individualized physical therapy treatment plans.
Provide skilled physical therapy services to short-term rehabilitation and long-term care residents.
Establish functional goals that promote mobility, strength, balance, endurance, and independence.
Collaborate with physicians, nursing staff, and the interdisciplinary team to ensure optimal patient outcomes.
Educate residents and families regarding treatment plans, mobility techniques, and discharge recommendations.
Complete accurate and timely documentation using Net Health and PointClickCare (PCC).
Maintain compliance with all federal, state, and company regulations.
Current Physical Therapist (PT) license in the Commonwealth of Pennsylvania.
Graduate of an accredited Physical Therapy program.
Skilled Nursing Facility (SNF) or Long-Term Care experience preferred, but new graduates are encouraged to apply.
Experience with Net Health and PointClickCare (PCC) is a plus.
Strong communication, clinical reasoning, and organizational skills.
Ability to work collaboratively in a fast-paced rehabilitation environment.
