Job summary

Provident Behavioral Health seeks a licensed Physical Therapist to deliver one-on-one care to older adults in private residences and Senior Living Communities in Iowa.

You will evaluate, develop personalized treatment plans, and provide evidence-based therapy to improve mobility, independence, and quality of life, documenting progress in the EMR and collaborating with PTs, OTs, SLPs, and Fitness Specialists. Telehealth options extend reach and flexibility.

Qualifications

• Valid PT license and degree from an accredited program.
• Proficiency with computers and EMR systems.
• Current CPR certification.
• Ability to meet physical demands, including lifting, repositioning, and driving.
