Job summary

DayToDay Health is seeking skilled Physical Therapists to provide individualized geriatric therapy through Geriatric House Calls™ in the community, delivering autonomous clinical interventions and striving for functional recovery. You will conduct 1:1 home-based PT sessions, tailor and adjust treatment plans, and collaborate with an interdisciplinary team to maximize patient outcomes.

Qualifications

• Valid Physical Therapy license in the practice state or eligibility to apply.
• Degree from an accredited physical therapy program.
• Basic computer literacy and CPR certification.
