Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist to deliver in-home care for clients. This part-time/per diem role offers meaningful, home-based therapy, including strength, mobility and safety training, with generous mileage reimbursement.

You will evaluate patients, design personalized plans, educate families, and document care using a mobile EMR, while collaborating with a supportive clinical team.

Qualifications

• Active MA PT license required.
• DPT or MPT from an accredited program.
• CPR certification required.
• 1+ year of home health experience preferred; new grads welcome.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to improve strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
