Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist to join our in-home care team. You will assess patients at home, design personalized treatment plans, and help clients regain strength, balance, and independence in familiar surroundings.

The position offers part-time or per diem scheduling, competitive pay of $75–$85 per hour with mileage reimbursement, and support from a skilled office team. New grads welcome with supervision available.

Qualifications

• Active MA PT license and CPR certification.
• DPT or MPT from an accredited program.
• 1+ year of PT experience preferred; new grads welcome.
• Reliable vehicle and valid driver's license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety
• Educate clients and families on exercises and home safety
• Collaborate with a multidisciplinary care team
• Document care using a mobile-friendly EMR
