Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist to perform in-home evaluations and deliver skilled interventions. This part-time/per diem role offers flexible scheduling and mileage reimbursement, with care plans tailored to restore strength, balance, and independence for patients in their homes.

The ideal candidate has an MA PT license (or greater), CPR certification, and reliable transportation to serve local visits.

Qualifications

• Active MA PT license is required.
• DPT or MPT from an accredited program is required.
• CPR certification is required.
• 1+ year of experience preferred; new grads welcome.
• Reliable vehicle and valid driver’s license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to improve strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile-friendly EMR.
