Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist to provide in-home PT services. You will evaluate patients, design personalized plans, and deliver interventions to improve strength, balance, mobility, and safety in the home setting.

Flexible scheduling and local visits are available, with mileage reimbursement and a supportive team. New grads welcome with appropriate credentials and a valid MA PT license.

Qualifications

• Active MA PT license.
• Graduated from DPT or MPT program.
• CPR certification is required.
• 1+ year of experience preferred; new grads welcome.
• Reliable vehicle and valid driver's license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to improve strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using a mobile EMR.
