BrightStar Care – Concord, Lexington & Woburn seeks a licensed Physical Therapist to deliver in-home care, evaluate patients, and create personalized plans to improve strength, balance and independence.

The role is part-time/per diem with flexible scheduling, mileage reimbursement, and a mobile EMR. Active MA PT license, DPT or MPT, CPR; 1+ year experience preferred; reliable vehicle required.

Qualifications

• Active MA PT license.
• DPT or MPT from an accredited program.
• CPR certification.
• 1+ year of experience preferred (new grads welcome)

Responsibilities

• Perform in-home PT evaluations and create personalized care plans
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety
• Educate clients and families on exercises and home safety
• Collaborate with a multidisciplinary care team
• Document care using mobile-friendly EMR
