Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist to provide in-home care. You’ll assess patients, develop personalized treatment plans, and help clients regain strength, balance, and independence in the comfort of their homes.

This part-time / per diem role offers competitive pay ($75–$85/hour) plus mileage, flexible scheduling, and supportive leadership. Active MA PT license, DPT/MPT, CPR, and at least 1 year of experience preferred.

Qualifications

• Active MA PT license required.
• DPT or MPT from an accredited program.
• CPR certification required.
• 1+ year of in-home PT experience preferred; new grads welcome.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile-friendly EMR.
