Job summary

BrightStar Care – Concord, Lexington & Woburn seeks a licensed Physical Therapist for part-time/per diem in-home care. Help patients thrive at home by improving strength, balance, mobility, and safety.

You will perform in-home evaluations, develop personalized plans, educate clients and families, and collaborate with a multidisciplinary team. A MA PT license, DPT or MPT, CPR certification, and reliable vehicle are required.

Qualifications

• Active MA PT license.
• DPT or MPT from an accredited program.
• CPR certified.
• 1+ year of experience preferred; new grads welcome.
• Reliable vehicle and valid driver’s license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using a mobile-friendly EMR.
