Job summary

Bardstown Health & Rehabilitation is seeking a compassionate Physical Therapist to join our team and provide exceptional patient care in a dynamic healthcare environment.

The ideal candidate will be licensed in Kentucky, CPR certified, and experienced with EMR documentation. You will assess patients, design rehabilitation plans, and collaborate with a multidisciplinary team to improve outcomes.

Qualifications

• Graduate from an accredited Physical Therapy program.
• Active Physical Therapy license in Kentucky.
• CPR certification required.
• Experience with PCC EMR is a plus, not mandatory.

Responsibilities

• Conduct Physical Therapy assessments of referred patients and provide direct patient care.
• Collaborate with other healthcare professionals to optimize patient rehabilitation plans.
• Document progress and maintain PCC EMR records accurately.
• Educate patients and families on home exercise programs and safety.

Qualifications:

• Graduated from an accredited Physical Therapy (PT) college or university.


• Valid Physical Therapy (PT) license for state practicing.


• Conduct Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.


• Proficient in EMR documentation (we use PCC but experience in this system is not required).


• CPR Certification.
