Job summary

Mt-Tabor-Health- is seeking a compassionate Physical Therapist to join a dynamic care team in Oregon. You will provide direct patient care, perform comprehensive PT assessments, and collaborate with colleagues to optimize rehabilitation outcomes.

Qualified candidates hold an accredited PT degree, an active PT license in the state, CPR certification, and familiarity with PCC EMR. This on-site role offers generous benefits and ongoing professional development.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Valid Physical Therapy license for the state of practice.
• CPR Certification.
• Proficient in EMR documentation (PCC).

Responsibilities

• Provide direct patient care and assess rehabilitation potential for referred patients.
• Conduct physical therapy assessments to evaluate medical conditions and functional capabilities.
• Document patient progress in PCC EMR.

Qualifications:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Valid Physical Therapy (PT) license for state practicing.
• Conduct Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
• Proficient in EMR documentation (we use PCC but experience in this system is not required).
• CPR Certification.
