TLC Care Center - NV is actively seeking a skilled Physical Therapist to join our team on a full-time basis, with PRN opportunities available. You will collaborate with physicians and staff to deliver evidence-based therapy, evaluate patients’ progress, and tailor treatment plans to maximize functional outcomes.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Licensed Physical Therapist in the state.

Responsibilities

• Perform physical therapy assessments of referred patients.
• Provide direct patient care to evaluate functional abilities.
• Collaborate with the care team to develop treatment plans.
• Document treatment sessions in PCC EMR.
• Participate in onboarding and ongoing professional development.

Qualifications:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Valid Physical Therapy (PT) license for state practicing.
• Conduct Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
• Proficient in EMR documentation (we use PCC but experience in this system is not required).
• CPR Certification.
