Henson Park Health & Rehabilitation is seeking a compassionate Physical Therapist to join our team in Kentucky. This role offers a $7,500 sign-on bonus and a pathway to collaborate with experienced healthcare professionals in a dynamic setting focused on high-quality patient care.

Qualifications

• Licensed PT in Kentucky and graduates from an accredited PT program.
• Demonstrated ability to assess patients and provide direct care.
• Familiarity with PCC EMR preferred; CPR certification included.

Responsibilities

• Conduct Physical Therapy assessments of referred patients, providing direct patient care to assess medical conditions and rehabilitation potential.
• Deliver patient-centered PT interventions and document progress in PCC EMR.
• Collaborate with the care team to optimize patient outcomes.

Qualifications:

• Graduated from an accredited Physical Therapy (PT) college or university.

• Valid Physical Therapy (PT) license for state practicing.

• Conduct Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.

• Proficient in EMR documentation (we use PCC but experience in this system is not required).

• CPR Certification.
