Job summary

Daleville Health & Rehabilitation is seeking a compassionate Physical Therapist to assess patients, design and implement rehabilitation plans, and deliver high-quality direct care on site. You will work with a multidisciplinary team to optimize functional outcomes and patient well-being.

The role requires a current PT license, DPT degree, CPR certification, and proficiency with PCC EMR. The position offers a supportive environment focused on growth and clinical excellence.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Licensed PT in state.
• Conduct Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
• Proficient in EMR documentation (PCC experience preferred).
• CPR certification.
