Job summary

Plaza-Health- is seeking a compassionate Physical Therapist to join our team in a dynamic healthcare environment. You will provide high-quality PT services, perform patient assessments, and document care in PCC EMR.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Valid PT license for state practicing.
• Experience conducting PT assessments and direct patient care.
• CPR certification up to date.

Responsibilities

• Provide direct physical therapy services to referred patients.
• Conduct assessments to evaluate medical conditions and functional status.
• Document care in the PCC EMR system (experience with PCC preferred).
• Collaborate with care team to develop and adjust treatment plans.

Qualifications:

• Graduated from an accredited Physical Therapy (PT) college or university.

• Valid Physical Therapy (PT) license for state practicing.

• Conduct Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.

• Proficient in EMR documentation (we use PCC but experience in this system is not required).

• CPR Certification.
