Job summary

Williamsburg Post Acute & Rehabilitation VA is seeking a compassionate Physical Therapist to deliver high-quality, patient-centered care in a welcoming setting. You will assess patients, develop treatment plans, and collaborate with our care team to restore function.

Qualifications

• Graduate of an accredited Physical Therapy program with a valid state license.
• Direct patient care experience and ability to assess medical conditions and rehabilitation potential.
• Experience documenting in an EMR; PCC experience preferred but not required.
• CPR certification required.

Responsibilities

• Perform PT assessments for referred patients to determine rehabilitation needs.
• Deliver direct patient care and develop individualized treatment plans.
• Document sessions accurately in PCC EMR.
• Participate in onboarding and ongoing professional development.

Qualifications:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Valid Physical Therapy (PT) license for state practicing.
• Conduct Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
• Proficient in EMR documentation (we use PCC but experience in this system is not required).
• CPR Certification.
