Job summary

UW Health’s Headache Clinic in Rockford, IL seeks a full-time Physical Therapist for a 100% FTE day shift. You will deliver patient-centered care, participate in ongoing CEUs, and contribute to a comprehensive benefits package including retirement, disability, and tuition support.

A $20,000 sign-on bonus is available for eligible candidates. Requirements include graduation from an accredited PT program and current IL licensure or eligibility, plus CPR certification.

• Participates in educational programs on an ongoing basis and complete mandatory CEUs.

Work Schedule:

100% FTE, Day Shift role. M- F 8am - 5pm. You may be eligible for a $20,000 sign on bonus, you will work at the Headache Clinic in Rockford.

Qualifications

Licenses & Certifications

Current license from the State of Illinois or in the process of acquiring licensure. An applicant for employment as a physical, occupational, or speech therapists must present acceptable evidence of licensure or authorization to test at the time of employment. An applicant who fails to provide this documentation will not be hired. A new employee will not be allowed to work as a therapist or assistant until evidence of licensure eligibility is provided. Required CPR certification. Required
