UW Health in northern Illinois seeks a licensed Physical Therapist to join the Home Health department in Rockford. The role is 100% FTE, full-time with Day/Evening shifts, Sunday through Saturday 08:00–17:00.

Qualifications include graduation from an accredited PT program and an Illinois license, CPR, and a valid driver's license. The position offers a comprehensive benefits package and opportunities for career growth.

Work Schedule

100% FTE, full-time. Day/Evening shift, Sunday - Saturday 0800 - 1700. You will work at the UW Health Home Health department.

Additional components of compensation may include:

• Evening, night, and weekend shift differential
• Overtime
• On-call pay

Qualifications

• Graduate of an accredited Physical Therapy program (or foreign equivalent as determined by the State of Illinois Department of Professional Regulation). Required

Licenses & Certifications

• Current license from the State of Illinois. Required
• Basic Life Support/CPR Certification. Required
• Must possess a current, valid driver's license issued by the individual's state of residence. Required
• An applicant for employment as a physical, occupational, or speech therapists must present acceptable evidence of licensure or authorization to test at the time of employment. An applicant who fails to provide this documentation will not be hired. A new employee will not be allowed to work as a therapist or assistant until evidence of licensure eligibility is provided. Required
