Job summary

Healthfirst is seeking a Pharmacy Navigator/Pharmacy Technician to review medication lists, coordinate refills, and communicate treatment plans with providers. You will help patients access medications, document care, and support MTM interventions.

Qualifications

• Minimum high school diploma or GED.
• Experience communicating clearly with patients and providers.
• Experience communicating on medication and medication issues.
• Nationally certified pharmacy technician or licensed/registered technician.

Responsibilities

• Review medication lists to remove barriers and support adherence as the member's Pharmacy Navigator.
• Facilitate refills, new prescriptions, appointments, and access to medications for financial issues.
• Coordinate MTM interventions with Healthfirst pharmacists and other partners.
• Monitor and communicate member adherence with community pharmacies.
