Job summary

Haven Home Care is expanding in Eastern Ohio and seeking an LPN to join as a supervisory per-visit role alongside another job. You will complete open visits, perform supervisory visits every 60 days, and conduct tests for aides.

Pay is per visit or task, with mileage reimbursement; there is no hourly wage or guaranteed weekly visits. We are building a brand-new territory across Ashtabula, Columbiana, Mahoning, and Trumbull counties.

Qualifications

• Must have Ohio LPN license in good standing.
• Current CPR certification.
• Reliable transportation and valid driver's license.
• Residence in Trumbull or Mahoning County preferred.
• Successful completion of criminal background check, skills test, and written nursing test.

Responsibilities

• Prepare accurate documentation per agency standards and regulatory requirements.
• Conduct regular supervisory visits every 60 days and open visits for new clients.
• Conduct skills tests and written tests for Home Health Aide candidates.
