Job summary

Haven Home Care Inc. in the Eastern Ohio expansion seeks an LPN to perform supervisory visits alongside another job. This is a per-visit role, not hourly, with pay per visit and per mile, plus mileage reimbursement.

Initial hours are minimal as the territory builds client base in Ashtabula, Columbiana, Mahoning, and Trumbull counties. The LPN will conduct open/supervisory visits, collect documentation, and administer pre-employment testing for aides.

Qualifications

• Must be at least 18 years old.
• Graduation from an accredited School of Practical Nursing.
• Current Ohio LPN license in good standing.
• Valid driver’s license and reliable transportation.
• Current CPR certification.
• Criminal background check, skills test, and written nursing test completed.
• Residence in Trumbull or Mahoning County preferred.

Responsibilities

• Complete open visits and supervisory visits every 60 days.
• Collect documentation per agency standards and regulatory requirements.
• Conduct pre-employment testing for Home Health Aide candidates.
• Assist with scheduling, documentation, and compliance tasks where required.
