Job summary

Haven Home Care Inc. is expanding its proven home care model to Eastern Ohio, seeking an LPN to perform supervisory and open visits on a per-visit basis. This role sits alongside another job rather than replacing it, with no guaranteed weekly hours.

You will be paid per visit or task (supervisory $25, open $35, skills test $35, written test office time $35) and mileage at $0.655/mile. Travel across Ashtabula, Columbiana, Mahoning, and Trumbull counties is required.

Qualifications

• Must be at least 18 years old.
• Graduation from an accredited School of Practical Nursing.
• Current Ohio LPN license in good standing.
• Valid driver’s license, reliable transportation, and current auto insurance for work.
• Current CPR certification.
• Criminal background check, skills test, and written nursing test required.
• Residence in Trumbull or Mahoning County preferred.

Responsibilities

• Complete open visits and collect documentation as required.
• Conduct supervisory visits to check on Home Health Aides every 60 days.
• Complete pre-employment testing for Home Health Aide candidates.
• Maintain accurate documentation according to agency standards and regulatory requirements.
