Job summary

Haven Home Care is seeking an LPN for a supervisory, per-visit role that sits alongside another job. Pay is per completed visit or task, with supervisory visits, open visits, skills tests, and office time tests, plus mileage reimbursement.

Initial hours are minimal as we build a brand-new territory in Eastern Ohio. The role involves supervising Home Health Aides, completing pre-employment testing, and coordinating visits across Ashtabula, Columbiana, Mahoning, and Trumbull counties.

Qualifications

• Must be at least 18 years old.
• Graduation from an accredited School of Practical Nursing.
• Current Ohio LPN license in good standing.
• Valid driver's license and auto insurance for work.
• Current CPR certification.
• Criminal background check, skills test, and written nursing test completed.

Responsibilities

• Documentation: Prepare accurate and thorough documentation per agency standards.
• Supervisory/Open Visits: Conduct visits every 60 days to check on Home Health Aides and complete open visits for clients.
• Pre-Employment Testing: Conduct skills tests and written tests for aides.
