Job summary

Qualifications

• BSN preferred
• 3-5 years of clinical nursing experience
• Hospital case management and care coordination experience preferred
• Associates Degree in Nursing required or higher

Responsibilities

• Facilitate patient care plans as outlined in the plan of care
• Lead daily care huddles to direct timely progress
• Collaborate with health care team to optimize resource use and patient satisfaction
• Assess admissions to ensure appropriate level of care and transitions
• Coordinate with payers and physicians on transitions and coverage
• Conduct assessments and interventions to reduce readmissions
