Job summary

Baystate Wing Hospital in Palmer, MA is seeking an RN Case Manager per diem to coordinate clinical care and ensure smooth transitions across settings. You will lead the interdisciplinary team in complex discharge planning and collaborate with payers to optimize outcomes.

The role requires a nursing license in Massachusetts, 3–5 years of clinical nursing and hospital case management experience is strongly preferred; BSN is preferred. Weekend/holiday commitments are part of the schedule.

Qualifications

• Associates Degree in Nursing is required.
• BSN preferred for this hospital case management role.
• Case management certification or related credential is valued.
• Active RN license in Massachusetts and hospital case management experience preferred.

Responsibilities

• Facilitates patients care plan as called for in the plan of care.
• Leads daily patient care huddles with nursing to direct timely progress.
• Cooperates with health care team to ensure efficient resource use and patient satisfaction.
• Assesses health care and transitional needs across care settings.
• Manages input from health care professionals and collaborates with payers.
