Baystate Health is seeking Hospital RN Case Managers to join our growing healthcare teams. The Nurse Case Manager coordinates clinical care, quality, and financial outcomes for a designated patient population, guiding efficient plans for community or post-acute reentry.

Leading the interdisciplinary team, the role develops complex discharge plans, collaborates with payers, and drives safe transitions. Massachusetts RN license and 3–5 years nursing experience are required, with BSN preferred.

Qualifications

• Requires current Massachusetts RN license and MLS.
• Experience in hospital case management or care coordination preferred.
• Strong understanding of utilization review and community resources.

Responsibilities

• Facilitate and lead the patient care plan as outlined in care plans.
• Lead daily patient care huddles with nursing to coordinate progress.
• Collaborate with health care team and payers to optimize resource use and outcomes.
• Ensure safe discharge planning and transitions with physician advisors.
• Conduct assessments to determine health care needs and post-acute plans.
