• Facilitates patients care plan as called for in the plan of care.
• Leads daily patient care huddles with nursing to direct timely progress.
• Directs appropriate use of hospital resources and ensures patient satisfaction.
• Conducts assessments to determine health care and transitional needs.
• Oversees admissions/continued stay using evidence-based criteria.
• Collaborates with payers and physicians to optimize transitions and reimbursements.

Education

Associates Degree in Nursing (Required)
