Job summary

Boston Medical Center is seeking an RN Care Manager Per Diem in the Case Management department. The role involves assessing, planning, implementing, monitoring, and evaluating options to meet patients’ health needs, using available resources to promote quality and cost-effective outcomes.

Collaboration with providers ensures timely and appropriate care across the continuum. The position is part-time, located in Boston, MA, and requires a BSN, MA RN license, and inpatient care management

Qualifications

• BSN from an accredited program and MA RN licensure.
• CCM certification preferred within 24 months of hire date.
• 1–3 years acute inpatient care management experience required.
• Inpatient Case Management experience required.
• Excellent interpersonal, verbal and written communication.
• Knowledge of managed care concepts and healthcare delivery systems.

Responsibilities

• Assess, plan, implement, monitor and evaluate options to meet health needs.
• Coordinate with providers and facilities to optimize care across continuum.
