Job summary

Baystate Noble Hospital is seeking an RN Case Manager per diem to coordinate clinical care, quality and financial outcomes for a designated patient population. You will lead an interdisciplinary team to develop complex discharge plans and ensure safe transitions for community or post-acute settings.

The role includes managing care plans, collaborating with payers and physicians, and supporting readmission reduction efforts.

Qualifications

• BSN preferred with 3–5 years of hospital case management or care coordination experience.
• Experience coordinating care plans and working with payers is strongly preferred.
• Ability to organize and prioritize workload to meet deadlines.

Responsibilities

• Facilitates patient care plans and leads daily care huddles with nursing.
• Oversees utilization of resources, ensures appropriate admissions and transitions.
• Collaborates with physicians, payers, and community resources to ensure safe continuum of care.
• Conducts assessments and participates in readmission risk management.
• Manages denials and works with physicians to optimize reimbursement.
