Job summary

Baystate Franklin Medical Center in Greenfield, MA is seeking an RN Case Manager Per Diem to coordinate clinical care, quality and financial outcomes for a designated patient population. The role leads the interdisciplinary team to develop complex discharge plans and ensure seamless transitions.

BSN preferred with 3–5 years of hospital case management or care coordination experience; strong knowledge of resources and payer/physician communications is required.

Qualifications

• BSN preferred; 3–5 years nursing experience in hospital case management or care coordination.

Responsibilities

• Facilitates patients care plan as called for in the plan of care.
• Leads daily patient care huddles with nursing to direct timely progress along the continuum.
• Works with the health care team to insure appropriate use of hospital resources and patient satisfaction.
• Manages input from health care professionals and conducts assessments for care needs.
• Evaluates admissions and continued stay using evidence-based criteria.
• Collaborates with payers through written and verbal communications.
• Leads discussions with physician advisors about safe transition plans.
• Directs care to other settings when appropriate.
• Collaborates with community case managers to ensure smooth continuum of care.
• Conducts assessments and interventions for patients at risk for readmissions.
• Manages denials and works with physicians to overturn for reimbursement.
