UC San Diego Health is seeking an RN Care Manager to coordinate care from pre‑admission to discharge at Jacobs Medical Center. The role emphasizes fast‑paced collaboration with physicians, nurses, social workers, and other providers to ensure cost‑effective, medically appropriate discharges.

The position requires an active CA RN license, BLS, and 3+ years in acute care management, with Epic and CarePort proficiency. Per diem, 8‑hour shifts, 20% appointment. Structured with growth opportunities.

Formación

• Must be a graduate from an accredited school of Nursing.
• Registered Nurse (RN) license issued by the State of California.
• BLS at time of hire with commitment to get BART within six (6) months of hire date.
• Minimum of 3 years acute care management experience.
• Demonstrated utilization review/discharge planning experience.
• Proficiency in Epic and CarePort.
• Effective interpersonal communication and negotiation skills, leadership ability, and conflict management and resolution.
• Thrives and excels in a fast‑paced, dynamic environment and committed to professional growth and ongoing departmental performance improvement.
• Demonstrated understanding of patient flow, and transitions to post‑acute care and community based services.
• Proficiency in information systems, databases, and programs including MS Office: Word, Excel, Outlook, and PowerPoint, etc.
• Must have fast and accurate typing, data entry skills and proficiency with detailed written communication appropriate for electronic medical records.

Responsabilidades

• Facilitate the patient's hospitalization from pre‑admission through discharge.
• Coordinate with physicians, nurses, social workers and other healthcare providers to expedite medically appropriate cost effective care.
• Apply clinical expertise and medical appropriateness criteria to resource utilization and discharge planning.
• Communicate with administration and the healthcare team as needed to facilitate patient care.
