Job summary

Qualifications

• Graduate of an accredited school of Nursing.
• California RN license required.
• BLS at time of hire; must obtain BART within 6 months.
• Minimum 3 years of acute care management experience.
• Utilization review/discharge planning experience.
• Proficiency with Epic and CarePort.

Responsibilities

• Facilitate patient hospitalization from pre-admission through discharge.
• Coordinate with physicians, nurses, social workers and others to expedite cost-effective care.
• Apply clinical expertise to resource utilization and discharge planning.
• Communicate with administration and healthcare team to support patient care.
