Job summary

UC San Diego Health in San Diego, CA, is seeking an RN Care Manager to facilitate hospitalization from pre-admission through discharge and coordinate care with physicians, nurses, social workers, and other providers. You will apply clinical expertise to ensure appropriate, cost-effective care and discharge planning.

This role requires a California RN license, BLS, and at least 3 years in acute care management. Epic and CarePort proficiency are preferred.

Qualifications

• Must be a graduate of an accredited school of Nursing.
• Registered Nurse (RN) license issued by the State of California.
• BLS at time of hire with commitment to get BART within six months.
• Minimum of 3 years of acute care management experience.
• Demonstrated utilization review/discharge planning experience.
• Proficiency in Epic and CarePort.
• Effective interpersonal communication and negotiation skills, leadership ability, and conflict management and resolution.
• Thrives in a fast-paced environment with commitment to professional growth.
• Understanding of patient flow and transitions to post-acute care and community services.
• Proficiency with MS Office (Word, Excel, Outlook, PowerPoint).

Responsibilities

• Facilitate the patient’s hospitalization from pre-admission through discharge.
• Coordinate with physicians, nurses, social workers and other providers to expedite appropriate, cost-effective care.
• Apply clinical expertise and medical appropriateness criteria to resource utilization and discharge planning.
• Communicate with administration and the healthcare team as needed to facilitate patient care.
