Job summary

UC San Diego Health is seeking an RN Care Manager to coordinate care from pre-admission through discharge, working with physicians, nurses, social workers and a multidisciplinary team to optimize length of stay and resource use.

The role requires an accredited nursing degree, California RN license, and experience in acute care management; proficiency with Epic and CarePort is essential. Strong communication and leadership skills are required.

Qualifications

• Must be a graduate of an accredited school of Nursing.
• Registered Nurse (RN) license issued by the State of California.
• BLS at time of hire with commitment to get BART within six months of hire date.
• Minimum of 3 years of acute care management experience.
• Demonstrated utilization review/discharge planning experience.
• Proficiency in Epic and CarePort.
• Effective interpersonal communication and negotiation skills, leadership ability, and conflict management and resolution.
• Thrives and excels in a fast-paced, dynamic environment and is committed to professional growth and ongoing departmental performance improvement.
• Demonstrated understanding of patient flow, and transitions to post-acute care and community based services.
• Proficiency in information systems, databases, and programs including MS Office: Word, Excel, Outlook, and PowerPoint, etc.
• Must have fast and accurate typing, data entry skills and proficiency with detailed written communication appropriate for electronic medical records.

Responsibilities

• Under general supervision, the RN Care Manager facilitates the patient's hospitalization from pre-admission through discharge.
• The Care Manager coordinates with physicians, nurses, social workers and other healthcare providers to expedite medically appropriate cost effective care.
• The Care Manager applies clinical expertise and medical appropriateness criteria to resource utilization and discharge planning.
• The Care Manager will communicate with administration and the healthcare team as needed to facilitate patient care.
