You will work with physicians, nurses and social workers in a dynamic department dedicated to transforming care for medically complex patients, with opportunities for leadership and professional development.

Qualificações

• Graduate from an accredited school of Registered Nursing.
• California RN license.
• BART or BLS at time of hire with commitment to obtain within six months.
• Minimum of 3 years inpatient RN care manager experience or equivalent with discharge planning and coordination.
• Utilization review/discharge planning experience.
• Knowledge of the payer industry, resource management, reimbursement, and evidence-based clinical practice.
• Proficiency in MS Office and data entry.
• Strong interpersonal, leadership, negotiation, and hospital operations knowledge.

Responsabilidades

• Facilitate the patient's hospitalization from pre-admission through discharge.
• Coordinate with physicians, nurses, social workers, and other healthcare providers to expedite care.
• Apply clinical expertise to resource utilization and discharge planning.
• Communicate with the administration and healthcare team as needed to facilitate patient care.
