UC San Diego Health is hiring a Per Diem RN Care Manager to guide patients from pre-admission through discharge in a busy acute hospital setting in San Diego, CA. The role requires strong clinical judgment, collaboration with care teams, and sound discharge planning to optimize outcomes.

Qualified applicants will hold a California RN license, have 3+ years of inpatient RN experience in acute care, and be proficient in InterQual and care-management software.

Qualifications

• Must be a graduate of an accredited school of Nursing.
• ADN RN with 3 years acute care manager experience accepted.
• California RN license required; BLS at hire with goal to get BART within 6 months.

Responsabilités

• Coordinate care from pre-admission through discharge for medically complex patients.
• Collaborate with physicians, nurses, social workers and others to expedite appropriate, cost-effective care.
• Apply clinical expertise and discharge planning criteria to resource utilization and patient flow.
