Qualifications

• California RN license.
• BLS at time of hire; obtain BART within six months.
• Minimum of 3 years acute care management experience.
• Proficiency in Epic and CarePort.
• Strong interpersonal communication, leadership and negotiation skills.

Responsibilities

• Facilitate the patient's hospitalization from pre-admission through discharge.
• Coordinate with physicians, nurses, social workers and other providers to ensure appropriate care.
• Apply clinical expertise to resource utilization and discharge planning.
